The size of your database and your sever performance are the most important things there. So if your MySQL server sometimes unable to complete the duplicate check query before the another feed gets pulled, the another copy of the same post may be created. Unfortunately PHP + MySQL is not very effective thing when you have to deal with a rather large database on a rather weak hardware.

Okay, for some reason the duplicates started to appear again, since yesterday.
Could it have something to do with the max. execution time of the script? It's currently set to the default cPanel settings of 30 seconds. If I raise it, will that have any effect?
I ask first, because I want to be cautious about changing this setting.

Well, same issue, same solution.
It appeared my host moved my site to a different server (with same specs) without consulting me, because the other server had been under attack. Only problem was: they installed my 64bit OS on a 32bit platform, which is asking for problems.
So I moved my sites to another server (4 cpu-cores and 4GB ram) and set the max. execution time to 2 minutes and now the duplicates issue is reduced to maybe 1 duplicate every few days (which probably happens, while the system is making automatic backups). So that is not a big deal.
